Residential In-Reach Service (RIR)

Our Residential In-Reach (RIR) service provides hospital type care, where appropriate, to people living in aged care facilities (RACF). Our RIR team is staffed by Clinical Nurse Consultants, Nurse Practitioners and medical staff from the Geriatric Medical Team. The RIR team visit residents in aged care facilities providing specialist assessment and treatment within the aged care facility which may help prevent an emergency department presentation or assist with care transition post a hospital admission. RIR is not intended to replace the care the resident receives from their usual doctor or treating practitioner.

Our Services

We can provide clinical assistance with, but not limited to, the following:

  • Comprehensive medical and nursing assessment
  • Medication reviews
  • Advance care planning and family meetings
  • Palliative care advice and referrals
  • Intravenous antibiotic treatment for chest infections and urinary tract infection
  • Delirium advice and assessment
  • Pain assessment and management

Service Delivery

  • 7 days per week service
  • Monday - Friday, 8am - 6pm
  • Saturday, Sunday and Public Holidays, 8am - 4pm

Eligibility criteria

  • Residents from aged care facilities within the City of Yarra, City of Boroondara and southern part of City of Darebin (Advice may be provided to facilities outside these areas on a case-by-case scenario).

Referral Process

We accept referrals from staff of Aged Care Facilities, General Practitioners and family members of aged care residents.
