Service overview

The Cancer Care @ Home Service is a program designed to provide anti-cancer treatments and supportive care therapies to eligible SVHM Haematology and Oncology patients in their home environment. The service aims to offer patients more choice in how, when and where they are treated.

The program is targeted at patients who are receiving their same day treatments through SVHM Cancer Services. Patients may receive some or most of their care in the home according to their individual regimens and requirements.

  • Specialist Cancer Nurses and District Nurses
  • Administration staff
  • Allied Health disciplines can be accessed for outpatient/phone follow up

Cancer Care @ Home Coordinator Support
Monday - Friday, 8am - 4:30pm

Cancer Care @ Home is supported by the Symptom and Urgent Review Clinic (SURC).

Out of Hours Support
Out of hours support is available through on-call Haematology/Oncology team via SVHM Switchboard (Ph: 9231 2211). 

Eligibility criteria

  • Catchment based, contact Cancer Care @ Home Coordinator for details
  • Adequate venous access or suitable access device (CVAD) insitu
  • Current Haematology/Oncology Cancer Service patient
  • Safe and suitable home environment for infusion delivery
  • Treatment duration <90mins
  • Eligible regimens selected are of a low adverse event risk. Patients need to have tolerated previous hospital administration of all related medications before commencing program.
  • Patient is clinically and medically stable

Referral process

  • Internal SVHM referrals are to be made via the Electronic Patient Journey Board
  • Screening of referrals will be ongoing for SVHM Cancer Services patients as they navigate changes in the treatments over time.
  • Cancer Care @ Home Coordinator can be contacted to help advise and enable referral process for eligible SVHM Cancer Service patients
