Community Rehabilitation Centres

Service overview

Community Rehabilitation Centres provide assessment and short-medium term rehabilitation programs for people who have a physical disability, are frail, chronically ill with a recent change in function or recovering from traumatic injury or illness. The primary focus is an interdisciplinary team approach with client agreed treatment goals.

Community Rehabilitation is run from centres at Kew and Fairfield.

Service delivery

  • Kew Community Rehabilitation Centre (CRC) - Monday to Friday 8:00 – 4:30
  • Fairfield Community Rehabilitation Centre – Monday to Thursday 8:00-4:30
  • Short- medium term length of stay (approximately 8 weeks)     
  • Sessions are delivered in either individual or group sessions

Eligibility criteria

  • Resides in the City of Boroondara, City of Yarra or City of Darebin.
    For home based services person must live south of Bell Street for City of Darebin.  
  • Must have identified and achievable goals and will benefit from short to medium term rehabilitation either as a result of an acute illness, injury, or surgery, or an exacerbation of a chronic condition resulting in a change in function.
  • Must be willing and able to actively participate in rehabilitation (medically, physically, cognitively, and psychosocially)
  • Community rehabilitation is the appropriate service to meet their needs and address their goals
  • Must be stable and have a GP willing to provide medical support
  • CRC clients must be willing and safe to travel into centre. Clients are encouraged to provide their own transport, but assistance with taxi costs can be provided.
  • Clients need to be living independently, with low level care supports
  • No identifiable risks to the client or staff of providing services
  • Rehabilitation does not include monitoring or maintenance or referrals for assessment only
