The GP Liaison Unit assists General Practitioners (GPs) to navigate the services provided by St Vincent’s Hospital. The team also works to improve communication between the hospital and general practice.

Receiving patient information

In the event that a General Practitioner has not received/does not have access to required patient information:

  • It may be available via My Health Record (MHR)
  • It can be requested from Health Information Services (HIS) via the form below, or practice letterhead including the required information.

National Health Services Directory (NHSD) - Health Direct
Maintaining up to date provider details in the National Health Services Directory (NHSD) – Health Direct is critical to receiving timely patient information as public health services use this database to obtain GP/practice contacts. Please include individual GP names and provider numbers. Provider numbers are not publicly listed on the NHSD website.

Routine transmission methods for clinical and administrative correspondence
Discharge summaries - HealthLink secure messaging (or fax if HealthLink not installed*)
Emergency Department e-Records - HealthLink secure messaging (or fax if HealthLink not installed*)
Specialist Clinic letters - Argus/HealthLink secure messaging or fax
Administrative notifications (e.g: acceptance/rejection of referrals, admission to hospital - fax)

*Please advise GP Liaison if you have HealthLink installed but are still receiving discharge summaries by fax. We will require your HealthLink Electronic Digital Identifier (EDI) please.

Statewide Referral Criteria
The Victorian Department of Health has worked with stakeholders to develop state-wide referral criteria (SRC) to improve the access to specialist clinics in public hospitals by improving the quality and appropriateness of referrals. SRC exist for a range of medical and surgical presentations and conditions - More information. SRC are incorporated into HealthPathways advice, where applicable.

HealthPathways Melbourne

HealthPathways Melbourne is an online portal designed to be used by general practice at the point of care to guide best practice assessment and management of common medical conditions, including when and where to refer patients. It is also available to medical specialists, nurses, allied health and other health professionals for use within their scope of practice.

The name HealthPathways reflects the referral pathways that assist in connecting patients to the right care, at the right place and with the right healthcare provider.
