The GP Liaison Unit assists general practitioners (GPs) to navigate the services provided by St Vincent’s Hospital. The team also works to improve communication between the hospital and general practice.
Clinic and Service Contacts for Referrers
St Vincent’s GP Liaison Unit were recently alerted to a significant number of referrals being directed to fax numbers not intended for this purpose, delaying processing and triage.
As a result we are providing a consolidated list of contacts that are likely to be most commonly used by referrers.
Please cross check against your address books and update where necessary.
Some specialist clinics have designated fax numbers. In these instances, it is preferable that referral be directed to the designated fax, but they will be redirected if sent to the General Specialists Clinic Fax: 03 9231 3489.

St Vincent’s rehabilitation support for patients with post COVID 19 complications
Many patients suffer prolonged physical, cognitive and functional impairments after COVID 19 infection, especially after ICU or other hospitalisation.
St Vincent’s is offering multidisciplinary rehabilitation at Kew or Fairfield sites or via Rehab-in-The-Home for patients in cities of Yarra, Darebin (South of Preston) and Boroondara. For surrounding suburbs, please enquire with the service.

St Vincent’s Hospital sends the following patient information to GPs following patient attendance and generally within the following timeframes:
- Discharge summary – within 48 hours
- Letter from Emergency Department – within 48 hours
- Letter from Specialist Clinics (initial appointment/discharge) – 5 working days
