The service assists clients to achieve their goals of care in their own home. It focuses on those who have activity and participation limitations resulting from recent or chronic impairments, where therapy can be safely and/or effectively delivered in a home environment. Team members Rehabilitation Physicians Nurses Physiotherapists Occupational Therapists Social Worker Dietitian Pharmacist Speech Pathologist Other Allied Health disciplines can be accessed as required Service delivery 7 days per week, minimum of daily sessions Program duration is usually 2 – 3 weeks Out of hours support accessible via St George’s Hospital After Hours Coordinator In person and telehealth sessions available Eligibility criteria Patients residing in the City of Yarra, City of Boroondara or City of Darebin. Catchment area is flexible and adjacent areas may be considered on a case-by-case basis The patient has achievable and realistic rehabilitation goals The patient’s goals are achievable within a 2-3 week admission The patient is clinically and medically stable Referral process External referrals with the completed referral form (attach referral form) can be made via fax 9231 8461 or email svhm.rehabathome@svha.org.au. Internal SVHM referrals are to be made via the Electronic Patient Journey Board. If patients choose to consult with you whilst under the care of this program, they are not eligible for Medicare rebates for services you provide as they are still considered inpatients of St Vincent’s Hospital Melbourne.
