With the support of Eastern Melbourne Primary Health Care Network’s CareSynergy program, St Vincent’s Hospital Melbourne is seeking to improve integration of patient care, increase access to specialist clinics services and build capacity in primary care. Resources to support ongoing care in General Practice include: Links to established guidelines and resources Draft GP Chronic Condition Management Plan (GPCCMP) Inserts – designed to serve as a base from which individualised, patient-centred management can be incorporated into the broader GPCCMP. Access to asynchronous advice for GPs looking after patients discharged via the Clinics2Community pathway. These resources are up to date at the time of publishing and will be reviewed periodically. Clinicians remain responsible for ensuring appropriate, up to date management plans for individual patients.
