Home Based Allied Health Services (Domiciliary) You are here:HomeHealth ProfessionalsCommunity and Aged CareHome Based Allied Health Services (Domiciliary/Rescare) The St Vincent’s Home Based Allied Health service is funded by the Commonwealth Home Support Program (CHSP) and Victorian Home and Community Care Program for Younger People (HACC-PYP). Our program focuses on maximising independence and wellbeing to support our clients with meeting their goals and remaining living at home. Team members The team includes: Physiotherapist Occupational therapist Speech pathologist Podiatrist Allied Health Assistant Dietitian Service delivery Our service is available to eligible clients living in the municipalities of Yarra, Boroondara and the southern part of Darebin (Fairfield, Northcote, Alphington and Thornbury). Eligibility criteria CHSP: Frail elderly clients who meet CHSP eligibility criteria; have a valid CHSP Referral code from My Aged Care; and are housebound (i.e. cannot drive or access transport to appointments). HACC: Adult clients under 65 years old who are housebound and meet HACC criteria. Referral process My Aged Care is the main access point for older clients seeking CHSP services. For HACC referrals, a written referral can be emailed to the address below.
