Aged Care Assessment Service (ACAS)

Service overview

ACAS completes comprehensive assessments for older clients in community and in hospital for eligibility for Aged Care Services Nationally.  These may include Residential care (permanent and respite), transition care (TCP- bed based or home based), Short term restorative care (STRC), Home Care Packages (HCP level 1-4) and Commonwealth  Home Support Program (CHSP)

North East Metro- St Vincent’s ACAS covers the geographical local government areas of Boroondara, Yarra and southern Darebin (south of Bells St).

ACAS and the regional assessment service (RAS) make up the assessment services for My Aged Care (MAC).

My Aged Care is a large data repository for Aged Care Services for the public. It is also the medical record for clients seeking aged care services where information and approvals can be shared between MAC assessment and service providers with client consent.

Team members

ACAS is a multi- disciplinary team including nurses and Allied Health that have specific training in completing comprehensive assessments. They are supported by Geriatricians.

Service delivery

ACAS completes face to face assessments in the client’s home, in hospital or residential care, by completing the National Screening Assessment Form and Support Plan. Approvals for Aged care types under The Act are listed in the Support Plan.

The duration of contact is usually a single assessment followed by referrals to services.

ACAS works business hours. The My Aged Care contact centre operates 8am to 8pm Monday to Friday and on Saturdays from 10 am to 2pm

The work of ACAS is controlled by legislation called the Aged Care Act

Eligibility criteria

Over the age of 65 with care needs that may require services to optimise function and maintain health.
