Rehabilitation in the Home is one of the Community Rehabilitation Services which provide assessment and short-medium term rehabilitation programs for people who have a physical disability, are frail, chronically ill with a recent change in function or recovering from traumatic injury or illness. The primary focus is an interdisciplinary team approach with client agreed treatment goals.
Rehabilitation in the Home (RITH) is a non-admitted (outpatient) service. It is available for people post hospital admission. The predominant focus for Rehabilitation in the home is supporting hospital flow.
