Cardiopulmonary Rehabilitation

Service overview           

Cardiac, Chronic Heart Failure, Pulmonary Rehabilitation.  Interdisciplinary program for people who have diagnosed heart and/or lung conditions (including N/STEMI, PCI/stent, CAGS, heart failure (all types), COPD, ILD, bronchiectasis, chronic asthma, etc).  Offers self-management education, exercise and motivational support to improve physical and psychosocial function and quality of life.

with some input from Dietitian, Occupational Therapist, Social Worker

Service delivery

In-person and telehealth options interchangeably depending on needs of person.  Fitzroy and Kew sites.  1-2 times per week frequency.  Average episode duration 1-2 months.  Team aim of zero-wait list.

Eligibility criteria

Diagnosed cardiopulmonary conditions by any medical professional.  Willingness to participate in service and open to learning new health and lifestyle strategies to improve own health.
