District Nursing

Service overview      

The St Vincent’s at Home (SVAH) District Nursing team assists local GPs and referring clinicians to manage client needs within the home setting.  Our team assists with medication management, diabetes monitoring and management, wound care, indwelling catheter care and general nursing assessment, whilst promoting client independence to manage their own care. 

The District Nursing service provides nursing care to clients under the Commonwealth Home Support Program (CHSP), Victorian Home and Community Care-Program for Younger People (HACC-PYP), Transition Care Program and is a service provider to Department of Veteran Affairs (DVA) eligible clients.

The SVAH District Nursing service also includes the Cancer Care at Home pathway, a key pillar of our health service Care Beyond the Walls strategy. Our team facilitates care for this client group including PICC/CVAD management and blood taking, subcutaneous and Intramuscular medication administration and chemotherapy disconnects.  Care is coordinated in conjunction with our Haematology, Oncology and Cancer Centre teams to provide treatment options to clients in the community.

Service delivery

  • 7 days per week
  • Staff available to visit once per day between the hours of 08:00 to 15:30
  • Referral coordination via our Intake Coordinator Team

Eligibility criteria

  • Clients need to be clinically stable and safe to be at home
  • Clients need to consent to our service and be agreeable for staff to visit in the home
  • Completion of an appropriate in-home visiting risk assessment

Referral process

  • Internal St Vincent’s Hospital Melbourne referrals are made and managed via the Electronic Patient Journey Board
  • External referrals:
    • CHSP – for clients over 65 via My Aged Care
    • Other clients – via our Intake Coordinator Team via phone 03 9231 3817 or email SVAHIntake.SVHM@svha.org.au
