GEM @ Home

Service overview

The GEM @ Home is a 7 day a week services that admits patients for short term geriatric medical assessment and management in the comfort of their home rather than in hospital. The service focuses on patients who have activity and participation limitations resulting from recent or chronic impairments and where therapy can be safely and/or effectively delivered in a home environment.

The service is targeted at patients with conditions associated with ageing, cognitive impairment and long-term illness.

Service delivery

  • 7 days per week, minimum of daily session
  • Program duration is usually 2 – 3 weeks
  • Out of hours support accessible via St George’s Hospital After Hours Coordinator
  • In person and telehealth available

Eligibility criteria

Patient resides in the City of Yarra, City of Boroondara or Southern City of Darebin.  Catchment area is flexible and adjacent areas may be considered on a case by case basis.

The patient requires multi-disciplinary management that includes geriatrician input.

The patient has achievable and realistic GEM goals.

The patient’s goals are achievable within a 2-3 week admission.

The patient is clinically and medically stable.

Referral process

Internal SVHM referrals are to made via the Electronic Patient Journey Board

GPs, please note, as this is an admitted service, GEM @ Home will provide all medical care to the patient at home. If patients choose to consult with you whilst under the care of this program, they are not eligible for Medicare rebates for services you provide as they are still considered inpatients of St Vincent’s Hospital Melbourne.
