Recently we redesigned our Endoscopy services at St Vincent’s Hospital Melbourne (SVHM). 

With the expansion of the National Bowel Cancer Screening Program (NBCSP), SVHM wants to ensure it can continue to match capacity with demand and deliver optimal patient outcomes.

The cornerstone of our new model of care places an important emphasis on General Practitioners (GPs) as the primary care providers for patients.

 

Key features of our model of ‘Shared-Care’ ~ strengthening the interface between primary and acute care

Any patient, who has undergone an endoscopic procedure and requires a repeat procedure in more than 12 months’ time, will be referred back to primary care for continued monitoring of his/her clinical condition in-between surveillance intervals. 

Our redesign approach is based on the principals of the latest evidenced-based primary health care research and lean six sigma change principals. A holistic collaborative approach was undertaken with our key stakeholders to ensure long term commitment and sustainability.
