Psychiatric Triage

Psychiatric triage provides advice and screening assessment for all new referrals to the service on a 24 hour, 7 days a week basis. All referrals for service must come via Psychiatric Triage.

Crisis Assessment and Treatment Service (CATS)

CATS offers psychiatric assessment and intensive community-based treatment to people in the acute phase of psychiatric illness.

Acute Inpatient Services

Adult Inpatient Service (AIS)

A 44-bed inpatient unit providing short term inpatient treatment to people during the acute phase of mental illness, including a six bed Extra Care Unit (ECU) for people with more intensive care needs, between the ages of 18 – 65.

The Victorian Aboriginal Health Service (VAHS) can access up to five beds for Koori consumers from across Victoria.

Normanby House Older Adult Inpatient Service

Comprehensive multidisciplinary assessment and treatment for older adults experiencing acute mental health distress.

There are 20 beds and access is facilitated by the Aged Persons Assessment and Treatment service. Referrals come from the St Vincent’s Hospital Emergency Department as well as other hospitals.

The programs listed below are provided across the adult community mental health program

Homeless Outreach Psychiatric Service (CHOPS)

This service provides acute assessment and case management for people with a mental illness who are homeless or at risk of homelessness.

Continuing Care Team (CCT)

Provides ongoing specialist care and support with the focus on meeting identified needs of clients through a range of clinical and therapeutic interventions. CCT may provide support at home, at the community mental health centres or over the telephone.

STAY (Supporting Tenancy At Yarra) Program

Secondary and primary assessments for tenants living in the Department of Health and Human Services public housing managed by the Fitzroy, Richmond or Collingwood Public Housing offices. STAY can be involved with tenants that are not current clients of the St Vincent’s Mental Health Service that are presenting with concerns about their mental health.

Early Psychosis Program:

A specialist response for people who are experiencing psychosis for the first time. The program is provided from within the other community teams and at all sites. The program aims to provide comprehensive assessment, flexible service delivery, family support, low dose medication and a strong focus on recovery.

Mobile Support and Treatment Service (MSTS)

MSTS supports people have higher support needs. Support is ongoing, intensive and people are usually seen where they live.

Brief Intervention Team

Provides access to a range of individual specialised support including psychology, occupational therapy, exercise physiology, nursing, dietetics or consumer peer support.

Community Group Program

Offers a range of therapeutic and recreational programs, both on-site at community clinics and in the community, aimed at bringing people together to support recovery, wellbeing & connection. Groups cover a range of different topics such as life skills, connecting with lived experience, psychology skills, health & wellbeing, and exercise & movement.

Complex Care

You may receive support from Complex Care if you have complex needs. The team can help you, your carers, and the teams supporting you to develop integrated and recovery focused care packages.

Short Term Community Outreach Services – Adult

HOPE (Suicide Prevention):Aim to reduce suicide attempts and associated psychological distress for clients, their carers and families through coordinated care and assertive outreach support for people who have had a suicide attempt and/or associated behaviours. Referrals are from within the MH service and from GPs/PHN/Ambulance service via Psychiatric Triage on 1300 558 862.

Extended Triage:Short term phone support following a client’s contact with St Vincent’s EDMH or psychiatric triage. Aimed at connecting clients with a GP, counsellor or other primary health care provider.

Community Residential Mental Health Services – Adult

Footbridge Community Care Unit (CCU)

The Footbridge CCU is a 20-bed residential rehabilitation program in a group of 10 units, and is staffed 24 hours a day, 7 days per week. CCU provides a shared living environment where people can be supported to achieve their recovery goals, manage their mental health and develop the everyday skills they may require to move on to their chosen living environment.

North Fitzroy Prevention & Recovery Care (PARC)

PARC is a short-term residential program with 10 beds, providing a shared living environment for people who don’t need to be in hospital but who are not quite ready to go home, or as an alternative to hospital.

PARC is a 24 hours a day, 7 days a week service.

Older Adult Mental Health & Wellbeing Services

The Older Adult Mental Health and Wellbeing program operates across the inner-city areas of Yarra and Boroondara and part of Darebin. In addition to community, inpatient and bed-based services, the Older Adult program provides support and secondary consultation for people residing in local aged care facilities.

Aged Persons Assessment and Treatment Team (APATT)

Assessment and treatment for older adults who are experiencing mental illness or psychological distress.

APATT works with the person, their family, GP and other service providers to ensure treatment and care meets the person’s needs. Care is usually provided where the person lives, and work to engage relevant community services is a priority.

A residential support function provides support and secondary consultation for people who reside in aged care facilities.

During acute phases of illness care may be provided at Normanby House for brief periods, while longer term treatment may be provided at affiliated psycho-geriatric residential care services (Berrengarra and Auburn House).

Residential Support Program (RSP)

Supports older adult healthcare providers by providing short-term intensive involvement that focuses on mapping behaviours of concern, using non-pharmacological approaches and assisting those who provide care to understand different diagnoses and how this is reflected in the consumer reactions to care. The Residential Support Program utilise a care plan to enable a clear and consistent supportive approach for residential care providers and family. Access to the Residential Support Program is via the APATT case manager.

Healthy Ageing Service (HAS)

The Healthy Ageing Service (HAS) is a primary health service that is funded by the Eastern Melbourne Primary Health Network (EMPHN)

The program provides service for people aged over 65 years (or 55 years if Aboriginal and/or Torres Strait Islander) living in the community or in a residential aged care facility with (or at risk of) mild to moderate mental health issues.

The multidisciplinary team provides early intervention through both primary and secondary consultation functions across Whittlesea, Nillumbik, Banyule and Boroondara.
