Community Rehabilitation is available to people living within Yarra, Boroondara and Darebin councils following a hospital stay or via a direct referral from the community. Criteria for Community Rehabilitation Services Clients must: consent to participate be medically stable have clear rehabilitation goals be cognitively able to actively participate in a rehabilitation program Maintenance; ongoing therapy or assessment only is not provided by St Vincent's Community Rehabilitation Services (please refer to local Community Health Centres). If uncertain, please call the Health Independence Program Central Intake on Ph: 1300 131 470; or fax referral to 03 9231 2202 and the team will contact you if required. Kew St George’s Hospital 283 Cotham Road, Kew 3101 Fairfield St Vincent's Community Services 440 Heidelberg Road, Fairfield 3078 Clinic Times Kew Monday to Friday 8:00am - 4:30pm Fairfield Monday to Thursday 8:00am - 4:30pm Community Rehabilitation Services are not open on weekends or public holidays. Costs There is an administrative fee of $7.00 for each attendance at the centre.
