The Young Adult Complex Disability Service (YACDS) started as a pilot project in 2004-2005. It became an ongoing program in 2006. We aim to help young adults with a complex physical disability, their families and carers, move into adult healthcare from children’s health services.

We help link those with a complex developmental disability who are transitioning from children’s health services into adult health services by; reviewing their current health needs and working with them to manage their own health care. We also provide a consultative service for young adults who have already transitioned into adult health care whose needs require a multidisciplinary review.

We offer a multidisciplinary patient centred approach, with team assessment and review, and individual or combined allied health intervention, as indicated. 


Our Team

Our Team consists of:

  • Occupational Therapists
  • Social Worker
  • Orthotist
  • Speech Pathologist
  • Dietitian

We are also able to access other allied health, nursing, medical and surgical services available at St Vincent's.

Our Services

We help adults manage their own healthcare, through:

  • Specialist medical and allied health assessment and support
  • Referral to other specialist services, within and external to St Vincent’s
  • Goal-directed therapy
  • Equipment assessment and advice, including wheelchairs, orthotics and home modifications
  • Establishing links to GPs and local therapists

The primary goal of the service is to link individuals with local health services.

Our Location

The service is based at St. Vincent’s Hospital, Fitzroy, with additional clinics operating regularly at St. George’s Hospital in Kew.      
Clinic appointments at Fitzroy are held on the ground floor of the Bolte Wing (Building F). At St George’s, the clinic appointments are held in the Community Rehabilitation Centre, ground floor (up one level from the underneath parking).

When you arrive please report to the reception desk. The reception staff will notify the appropriate YACDS staff member that you have arrived and provide you with further direction as required.

Parking near the Building F - Bolte Wing can unfortunately be difficult at times. 

A patient drop-off area is available in front of the Nicholson Street entrance to the Bolte Wing and to the rear of the Bolte Wing via the boom-gated driveway.

Clinic Times

Multidisciplinary clinics run most Monday’s and alternate Thursdays between 9.00am – 4.00pm

Individual medical and allied health appointments can be arranged on other days.

The service operates on a part time basis.

Contact Us

Our service is a public outpatient service. You can self-refer, but for medical consultations we require a referral from a GP. Please speak to your GP or current treating specialist if you believe you would benefit from a referral to our clinic.
