The Geriatric Evaluation Management (GEM) Unit provides assessment and treatment of conditions associated with ageing. The Unit aims to help achieve maximum independence and quality of life. It also provides a short-term home-based service which assists people to manage better in their own homes.
Where is the Aged Care Unit?
The GEM and associated home-based service is located on level 3, Bolte Wing, St Vincent's, Melbourne.
How can an aged care program help?
After admission, members of the GEM team assess clients' needs and establish goals. The team may include medical and nursing staff, social workers, occupational therapists and physiotherapists. Staff from other disciplines may also be involved.
Regular team meetings are held to monitor clients' progress. Meetings are also arranged with family and carers to plan for the time following discharge.
Caritas Christi Day Centre/Planned Activity Group (PAG)
This Caritas Christi Day Centre/PAG is part of the Aged Care Services at St Vincent’s. Groups meet at the Caritas site in Kew 4 days per week.
