REQUEST PATIENT INFORMATION

Health Information Services (Medical Records) are able to provide patient information to treating medical practitioners and allied health staff.  If the practitioner is not listed as the current treating Doctor, patient consent should be sought.  Patient information is released in accordance to the SVH Privacy and Confidentiality policy. These requests can be initiated via phone, fax or post.

To make it easier to locate patient records the following patient details should be given:

 - Full name

 - Date of birth

 - Address

 - St. Vincent's UR number (if available)
