REHABILITATION AND DOMICILIARY SERVICES

 

Community Rehabilitation Centres (CRC)
Short term rehabilitation in a community outpatient setting for patients requiring physical or neurological rehabilitation. An initial assessment is performed to determine individual eligibility.

Domiciliary Team Service
Provides multidisciplinary assessment and management at home. Services include occupational therapy, physiotherapy, speech pathology, podiatry and nutrition. This service is for patients housebound due to physical or psychological conditions.

Rehabilitation in the Home
Provides a short-term goal-based, intensive rehabilitation at home. Services include physiotherapy, social work, occupational therapy and speech pathology. This service is available to patients over 18 years who are unable to access outpatient rehabilitation services.

Rescare, Employed Carers Day Respite Program

Provides respite for the carers of frail older persons. Clients must live or work in the Eastern area of Melbourne. The service offers nursing care, health assessments, personal care, wound care and a comprehensive activities program for clients with high needs, in a secure environment, located at St Georges Hospital, Kew. Access to Continence and other clinics facilitated.

 

Open Monday – Friday 0730-1800.

 

Aged care assessment is not essential.
