CHRONIC DISEASE MANAGEMENT

 

St. Vincent's Hospital Melbourne provides a range of programs which specifically target patients with chronic and complex diseases. The programs utilise evidence based chronic disease management and comprehensive case management.

ALERT

Assessment l Liaison l Early Referral Team

 - Identifies patients coming through the emergency

   department who are at risk of multiple hospital

   presentations

 

 - Target groups include chronic disease, disability, drug

   and alcohol issues, homelessness and mental health

 

 - Focused on service linkage and care coordination

 

 - Refers patients to other St. Vincent's chronic disease

   management programs

 - a rapid assessment clinic

 - co-ordinated care whilst in hospital or clinics

 - patient education

 - exercise programs
