HARP provides chronic disease and complex care management for your patients who are at risk of presenting to hospital.

HARP can provide:

• rapid aged care assessment (Treatment, Response & Assessment for Aged Care or TRAAC)
• chronic disease management (Restoring Health: rehabilitation for heart failure, chronic lung disease and diabetes and health coaching for coronary artery disease)
• care of the person with complex psycho-social needs (Assessment, Liaison & Early Referral Team or ALERT)
