St Vincent's Residential Aged Care In-Reach Service
| St Vincent's Residential Aged Care In-Reach Service is available to all patients from selected residential aged care facilities.
If a GP or locum is not available to visit the aged care facility, St Vincent’s Aged Care In-Reach Service can:
- offer phone advice regarding clinical problems to aged care staff
- provide on-site assessment of unwell patients if required
- provide on-site treatment in collaboration with the GP if required
- provide triage for possible on-site emergency physician assessment and/or treatment
- help prevent an unwanted trip to the Emergency Department
- fast track aged care residents through the Emergency Department of St Vincent’s
