Aged Care Assessment Service (ACAS)
The role of the Aged Care Assessment Service (ACAS) is to determine the overall care needs of frail older people (over 70 years of age) and to assist them to gain access to the most appropriate types of services. In doing this ACAS aims to:
- comprehensively assess older people taking account of their restorative, physical, medical, psychological, cultural and social needs
- involve clients, their carers and service providers in the assessment and care planning process
- provide access to case managed packages of care for people with complex needs
- provide approval for Commonwealth funded aged residential and respite care
GPs are particularly encouraged to contribute to the planning process and ACAS welcomes their referrals, participation and input. |
