Aged Care Assessment Service (ACAS)
Provides aged, cognitive, physical, social and emotional assessment for
community aged care packages (CACPs) and residential care placement. Patients
aged 65 years and over or patients suffering significant age related conditions
and living in St. George’s Hospital catchment area are eligible for this
service.

Treatment, Response, Assessment for Aged Care (TRAAC)
TRAAC
is a short-term rapid service for older people living in the community or residential care
who are experiencing a crisis which might lead to hospitalisation. The TRAAC
program provides alternative options that enable people to be supported where
they live. The service provides:
- aged care assessment
- care planning
- case
management
- home support services

geriatrician consultation
The
TRAAC Program accepts referrals from any source including community agencies and
GPs. Clinicians are located at the St. George's Aged Care Assessment Service
(ACAS) and St. Vincent's Hospital Emergency Department. |
