St. Vincent's Hospital Melbourne provides a range
of programs
which specifically target patients with chronic and/or complex diseases. The
programs utilise evidence based chronic disease management and comprehensive
case management. GP
referrals to all programs are welcomed.
Restoring Health
Designed for patients with chronic lung disease, heart failure or
diabetes, the program supports patient with access to:
- a rapid assessment
clinic
- co-ordinated care whilst in hospital or clinics
- patient education

exercise programs
- co-ordination with community services
- community pharmacy
liaison program
COACH
The
COACH program is a telephone support and education program for patients with
coronary heart diease. A health professional (Coach) aims to empower patients to
drive the process of achieveing and monitoring target levels of their identified
risk factors.
Assessment, Liaison and Early Referral (ALERT)
ALERT
is focused on enhancing service linkage, care planning and continuity of care
for clients with complex health and social needs. The key target groups include
disability, drug and alcohol issues, homelessness and mental health.
