About us
The GP-Mental Health Liaison Service is an important part of St Vincent’s Mental Health's commitment to meeting the needs of General Practitioners in the cities of Yarra and Boroondara. Our team is made up of an Occupational Therapist and Clinical Psychologist.
The Service has an overarching aim:
- To ensure a seamless a continuum of care for mental health patients as they move between GP practices and the hospital systems.
The Service has two primary functions:
Engagement
To promote an effective working relationship between St Vincent’s Mental Health and GPs so that patients receive timely and effective care
Communication
To enhance effective communication protocols between St Vincent’s Mental Health and GPs to ensure a seamless continuum of care for patients
