GP LIAISON UNIT

St. Vincent’s Health GP Liaison Unit provides a consultancy service across St Vincent’s Health to develop and improve the interface with General Practitioners. Fundamentally the unit’s function is to ensure  systems support the interface between the Health Service and General Practice and promote the continuum of patient care.

Key objectives of the unit are to:

  • Maintain relationships and work collaboratively with local Divisions of General Practice
     

  • Promote awareness of GP needs and feedback across SVH
     

  • Assist GPs to better understand and navigate SVH services by publicising key programs and information relevant to General Practice.
     

  • Respond to opportunities in the healthcare environment which can enhance patient care

As demand will be high for appointments when Clinics re-open in the New Year, we would like to remind you that all referrals require detailed clinical information and complete demographic details to be triaged:

Patient details including:

- mobile phone number

- Reason for referral & duration of symptoms
- GP details

- Management to date and response to treatment
- DOB

- Relevant pathology and imaging reports
- Interpreter requirements

- Medications
- Past medical history
